Albuterol: Warrick Pharmaceuticals

URGENT DRUG RECALL ALERT

Subject: Warrick Pharmaceuticals' Recall of Albuterol. Warrick Pharmaceuticals has issued a voluntary recall on a single lot of its generic albuterol metered-dose inhaler aerosol 17g. This recall affects LOT # 9-BBS-525 which was distributed between late April and early May 1999. The inhalers have an expiration date of August 2001. Fewer than 150 canisters are reported to be affected. According to a Reuters article issued September 10, 1999, Warrick said that it considers the matter serious and that it merits immediate attention. It cautioned that use of the product "may cause adverse health consequences requiring medical treatment." Subject: Hollister-Stier Laboratories recall of Ana-Kit®. As of August 27, 1999, Hollister-Stier Laboratories LLC is voluntarily recalling specific lots of the Ana-Kit® Anaphylaxis Emergency Treatment Kit. The company is taking this action after being notified by Wyeth-Ayerst Laboratories (U.S.) that they are voluntarily recalling several lots of their Epinephrine Injection product that is used in the Ana-Kit®.

The Ana-Kits® (Item #6800ZA; DIN 02163950) distributed between January 1998 and April 1999 with the following lot numbers (found on the front label of the kit) should be returned to the local pharmacy for replacement:

It is important to note that Ana-Kits® will have the Bayer Inc. logo on the front label of the packaging. Bayer Inc. marketed Ana-Kit® in Canada prior to selling its allergy portfolio to Hollister-Stier Laboratories in April, 1999.

The Epinephrine Injection (USP 1:1000 in 1 mg/ml Tubex®) used in the Ana-Kit® is being recalled after Wyeth-Ayerst Laboratories (U.S.) observed discoloured samples in routine quality-control tests. A few of these discoloured samples were found to be sub-potent, meaning the product might not be 100 per cent effective in an emergency.

While only these specific lots of the product are affected by the recall, consumers are encouraged to consult their pharmacist to discuss the need for replacement product. Should a replacement product not be readily available, patients are instructed to contact their physician or pharmacist immediately for advice regarding alternative medication.

If an emergency occurs prior to the product being replaced, patients are being advised to use the product and seek medical attention. Kraft Canada is advising consumers allergic to almond and/or pistachio nuts that four flavours of Jell-O Instant Pudding products with the code numbers listed below, sold in stores across Canada, may potentially contain almond or pistachio nuts. The code number is imprinted on the side panel. The four flavours of Jell-O Instant Pudding affected are:

This alert follows a product recall announced Thursday, November 25, 1999. Based on information available at the time, Kraft Canada moved quickly to recall Jell-O Lemon Instant Pudding (code number 92642) from a limited number of stores in Ontario. Based on further investigation, we are taking the extra precaution of expanding our recall to these four flavours, sold nationally. We continue to believe only a very small amount of product may contain almond and/or pistachio nuts.

We have also contacted the Canadian Food Inspection Agency and they agree with our course of action.
